Scott Forman, M.D., is a board-certified ophthalmologist specializing in general eye care and neuro-ophthalmology. Dr. Forman provides general and comprehensive ophthalmology exams. The doctor brings dedicated expertise to providing the highest quality approach to eye health, staying on top of the latest advancements in his field. When Scott is off the clock, he taps into his passion for playing blues and jazz tunes on his guitar or cycling around town with his wife, taking in the beautiful Pioneer Valley.
Education
Dr. Forman earned his medical degree from the prestigious Robert Wood Johnson Medical School. He completed his Ophthalmology residency at N.Y. Medical College, New York, NY. Dr. Forman completed a Fellowship in Neuro-Ophthalmology at Columbia Presbyterian Medical Center and Manhattan Eye, Ear & Throat Hospital.
